Admission Criteria for Services at PLACES, Inc.

There are separate admission criteria for each of PLACES three services lines, the Residential Program, the Supportive Living Program and the Permanent Supportive Housing Program. Residential Program - Adult Care Facilities

All candidates for placement in a PLACES, Inc. ACF must meet the following criteria:

  • Meet the Ohio Department of Mental Health's definition of severely and persistently mentally ill
  • Be a resident of Montgomery County aged 18 or older
  • Be linked with one of these ADAMHS Board's contract agencies - CAM, Day-Mont BHC, Eastway Corp. or South Community. PLACES cannot house clients linked to New Dimensions.
  • Have a Community Support Specialist (CSS) from an ADAMHS Board contract agency (listed above)
  • Meet the ADAMHS Board's criteria for Residential Care.

*Individuals with histories of violence and/or substance abuse disorders will be considered on a case-by-case basis.

Referral Process: A Community Support Specialist must submit a referral to Residential Support. Only referrals from Residential Support are accepted.

Supportive Living Program

All candidates referred by ADAMHS agencies must meet the following criteria:

  • Meet the Ohio Department of Mental Health's definition of severely and persistently mentally ill
  • Be a resident of Montgomery County aged 18 or older
  • Be linked with one of these ADAMHS Board's contract agencies - CAM, Day-Mont BHC, Eastway Corp. or South Community.
  • Have a Community Support Specialist (CSS) from an ADAMHS Board contract agency (listed above)
  • Demonstrate a need for SLP service as documented during intake

All candidates referred as qualifying for Opening Doors for the Homeless must meet the following criteria:

  • Meet the HUD definition of homeless
  • Have a mental health or substance abuse diagnosis provided by an individual qualified to diagnose
  • Be receiving services from at least one other Opening Doors partner agencies - The Other Place, Salvation Army Booth House, Eastco, Samaritan Homeless Clinic and CADAS

Referral Process: SLP accepts referrals from Community Support Specialists, homeless outreach workers, residential treatment providers, and individuals. To submit a referral, mail or fax the required form to PLACES, Inc. Once the referral is received, an intake will be arranged at the convenience of the referring party and/or the prospective client.

"Housing First" Permanent Supportive Housing Program

Potential tenants must meet the following criteria:

  • Single adults aged 18 or older who meet HUD's definition of chronic homelessness
  • Have a diagnosis of severe and persistent mental illness as defined by HUD and documented by Crisis Care or the Samaritan Homeless Clinic
  • Clients for Cobblegate may not be linked with the public health system
  • Tangy Court can accept clients linked with the public health system

Referral Process: PLACES accepts referrals for this program from any source. Referrals typically come from the homeless provider network, and primarily from the shelters, but individuals may self-refer. To submit a referral, mail or fax the required form to PLACES, Inc.
