Referring Someone to the Housing First Program

Tenants accepted into Cobblegate, Tangy Court, Belvo or Imperial Court must meet certain criteria.
Prospective tenants must:
- Meet the U.S. Department of Housing and Urban Development (HUD) definition of homeless (preference is given to individuals who are chronically homeless)
- Provide a diagnosis consistent with a serious mental illness as defined by HUD
- Need the level of services provided by the Housing First Program as determined by the Homeless Solutions Policy Board’s centralized assessment process (Front Door)
- Be willing to sign and comply with the terms of a standard lease
Documentation required for tenants:
- Front Door assessment and referral from the Homeless Solutions Policy Board
- Homelessness verification
- Most recent diagnostic assessment (if available)
- A completed Authorization for Release of Information
- Photo identification
- Social Security card
- Birth certificate
- Proof of medical insurance (if any)
- Proof of residency (Montgomery County)
- Proof of income
Referral process:
- Referrals must come from the Homeless Solutions Policy Board’s centralized assessment process (Front Door) and cannot be made directly to PLACES.
- Individuals may access the Front Door process through any of the four gateway shelters:
- St. Vincent de Paul Gettysburg Gateway for Men
- St. Vincent de Paul Gateway Shelter for Women and Families
- Daybreak’s shelter
- YWCA’s domestic violence shelter
